Job Description:
A company is looking for an RN Case Manager to join their team
on a contract basis.
Key Responsibilities
Evaluate individual member needs and develop personalized care
plans
Collaborate with healthcare providers and stakeholders for seamless
care delivery
Track member progress and adjust care plans to optimize
outcomes
Qualifications & Requirements
Active Compact State RN License
At least 2 years of recent Case Management experience in a Health
Plan
Experience with Special Needs Plans (SNP)
Strong computer skills, particularly in Microsoft Office
Bilingual in English/Spanish preferred; CCM certification is a
plus
